Supervise staff and provide a meaningful day for the clients.\rESSENTIAL FUNCTIONS AND RESPONSIBILITIES:\r\r\rSupervise, train, and provide guidance to the staff in accordance with state and federal regulations, Benchmark Human Services personnel policies, the Supported Living policy, and procedure manual.\r\r\rParticipates as part of an interdisciplinary team in the development of each client’s Individual Support Plan (ISP) and Person Centered Plan (PCP).\r\r\rParticipate in direct service for each client.\r\r\rComplete daily, weekly, monthly, and yearly documentation as required by Benchmark and state and federal regulations. Maintain in-home client files according to standards and protocol established.\r\r\rFacilitate all repairs and required maintenance of the client’s home.\r\r\rResponsible for following client’s budget and ensuring payment of bills. Assist client in maintaining Medicaid eligibility by scheduling and attending all Medicaid appointments and providing proper documentation.\r\r\rCommunicate professionally with families, Case Manager, Behavioral Consultant, health care personnel, schools, landlords, employers, etc.\r\r\rParticipate in SL Manager’s meetings, team meetings and other departmental meetings, ISP, Quarterlies, IEP meetings, and other meetings as assigned.\r\r\rImplement training programs as specified in each client’s ISP.\r\r\rComply with behavior support policies and procedures and implements approved behavior support programs.\r\r\rParticipate in the hiring and discipline of assigned staff.\r\r\rEnsure proper medication/treatment administration and assist in meeting the client’s health care needs.\r\r\rSchedule and assist clients in all medical appointments and procedures. Ensure completion of proper follow up.\r\r\rPlan and implement community-based, age appropriate, recreational activities for the clients.\r\r\rImplement required training protocol for new and existing staff.\r\r\rRespond appropriately and promptly to on-call situations.\r\r\rReport any suspected abuse, neglect, or exploitation to supervisor or department head.\r\r\rMaintain employment-required documentation (current auto insurance, driver’s license), certification (i.e., First Aid, CPR, Medication Administration, Mandt, etc.), and training (OSHA, Abuse & Neglect, etc.) as required to comply with federal, state and program requirements and standards.\r\r\rPerforms other duties as assigned.\r\r\rMust maintain good attendance throughout employment.\r\r\rOther duties as assigned.\r\r\rComply with all standards to assure the health and safety of all individuals.\r\r\rReport to work as scheduled.\r\r\rKNOWLEDGE, SKILLS, AND ABILITIES:\rQUALIFICATIONS:\r\r\rBachelor’s degree preferred. \r\r\rTwo years experience working with individuals with mental disabilities.\r\r\rA minimum of 1-year supervisory experience preferred.\r\r\rValid CPR and First Aid.\r\r\rValid driver’s license and auto insurance.\r\r\rMust complete training as required: Interviewing, hiring, discipline, employee relations, etc\r\r\rThe above declarations are not intended to be an all-inclusive list of the duties and responsibilities of the job described, nor are they intended to be such a listing of the skills and abilities required to do the job.
