\u003cp\u003eThe successful candidate will have unwavering compassion, an open mind in terms of pathways to recovery/harm reduction with respect to substance use disorders, demonstrated expertise with housing navigation and tenancy preservation, and extensive experience assessing and addressing social determinants of health.\u0026nbsp; Working knowledge of area resources/providers/quality assurance platforms/utilization \u0026amp; review methodologies are a plus.\u0026nbsp;\u003c/p\u003e

\u003cp\u003eProvides necessary training, consultation and support to subcontractor/partner agencies.\u003c/p\u003e

\u003cp\u003eEnsures comprehensive assessments, screening/outcome measures, and support plans for assigned clients are trauma informed, strengths based, and reflect identified needs and treatment preferences of the person served through supervision and chart/utilization review process. \u0026nbsp;\u003c/p\u003e

\u003cp\u003eAssess client needs and capacity to make independent living decisions, including risk assessments for clients around safety and tenancy preservation, and seek consultation if needed to determine short/long term intervention(s) required to ensure safety and quality of life. Ensures \u0026ldquo;warm handoff\u0026rdquo; to new provider if services are intensified.\u003c/p\u003e
